Psychotherapeutic Session Plan (30 Minutes)
Session Objective
To help the patient manage their anxiety and challenge negative thoughts related to the belief that "no one likes me" using Cognitive Behavioral Therapy (CBT).
Session Outline
1. Welcome and Warm-Up (5 minutes)
- Greeting: Begin with a warm and welcoming tone.
- Check-in: Ask the patient how they have been feeling since the last session or in the past week, focusing on their anxiety.
2. Psychoeducation on Anxiety (5 minutes)
- Understanding Anxiety: Briefly explain what anxiety is and how it can manifest in thoughts and feelings. Emphasize that it's a common experience.
- Cognitive Distortions: Introduce the concept of cognitive distortions, particularly focusing on the idea of "all-or-nothing thinking" as it relates to feeling disliked.
3. Identifying Negative Thoughts (5 minutes)
- Thought Record Exercise:
- Ask the patient to identify specific thoughts contributing to their anxiety, especially around the belief of "no one likes me."
- Encourage them to write down those thoughts or share them verbally.
4. Challenging Negative Thoughts (10 minutes)
- Cognitive Restructuring:
- Guide the patient to question the validity of their thoughts. Use the following prompts:
- What evidence do I have that supports or contradicts this thought?
- Are there any facts that I’m overlooking?
- How would I advise a friend who had this thought?
- Encourage the patient to develop alternative, balanced thoughts. For instance, "I may not be liked by everyone, but there are people who care about me."
5. Coping Strategies (3 minutes)
- Breathing Techniques:
- Teach simple breathing techniques to help manage anxiety, such as the 4-7-8 technique:
- Inhale for 4 seconds, hold for 7 seconds, and exhale for 8 seconds.
- Grounding Techniques:
- Introduce the “5-4-3-2-1” technique to help the patient ground themselves in the present moment:
- Identify 5 things they see, 4 things they can touch, 3 things they can hear, 2 things they can smell, and 1 thing they can taste.
6. Wrap-Up and Homework Assignment (2 minutes)
- Summarize: Briefly review what was covered in the session.
- Homework:
- Ask the patient to practice using the thought record tool for a week, noting instances when they feel anxious and the thoughts associated with those feelings.
- Encourage them to practice the breathing and grounding techniques whenever they feel anxiety rising.
Conclusion
- Encouragement: Remind the patient that progress takes time and that it's okay to reach out for support.
- Next Appointment: Confirm the next session time and suggest to bring any thoughts or experiences from homework for discussion.
This structure offers a concise and focused approach within a 30-minute session frame, tailored to the patient's anxiety and beliefs.
