Session Structure
1. Introduction (5 minutes)

2. Emotional Exploration (10 minutes)
- Facilitating Emotional Release:
- Encourage the patient to express their sadness verbally. Ask open-ended questions, such as:
- "Can you describe where you feel this sadness in your life?"
- "What situations recently have contributed to these feelings?"
- Allow the patient to share thoughts and emotions without interruption. Make sure they're aware that it’s okay to feel sad and that expressing this emotion is valid.
Cognitive Restructuring (10 minutes)

- Introduce the concept of cognitive distortions. Explain that thought patterns like “I am responsible for everything that goes wrong” can lead to feelings of sadness and overwhelm.

4. Challenge Unhelpful Thoughts
- Encourage Re-evaluation of Responsibility:
- Ask the patient to identify specific instances where they felt responsible for everything. Questions may include:
- "Can you think of a time when you felt responsible for a situation that was clearly beyond your control?"
- "What other factors may have contributed to that situation?"
- Guide them to reframe these thoughts; for instance, “It’s healthy to take responsibility for my actions, but I cannot control everything.”
Integration and Closure (5 minutes)

- Revisiting Key Points: Summarize the session by highlighting key insights about the nature of their sadness and the cognitive distortions identified.
- Encourage Self-Compassion: Remind the patient that it’s okay to feel sadness and that they are not alone in their experiences. Promote self-compassion by suggesting they be kinder to themselves regarding feelings of responsibility.
6. Homework Assignment (Optional)
- Reflective Journaling: Ask the patient to keep a journal for the week, noting any instances where they feel overwhelmed by responsibility. Encourage them to reflect on alternative perspectives during these moments. This can help build awareness of their thought patterns honed during the session.
7. Close the Session
- Thank the patient for their openness and vulnerability. Encourage them to carry forward what they've learned today and remind them of the importance of self-care.
