Objective:
To create a safe space where the patient can express their anxiety, explore underlying feelings, and begin to develop coping mechanisms to address their anxious mood.
Session Overview:
This session will focus on building rapport and trust while gently guiding the patient to participate in the therapeutic process. The therapist will employ techniques from cognitive behavioral therapy (CBT) and mindfulness to help the patient manage anxiety.
Session Structure
Introduction (5 Minutes)
- Welcoming the Patient: Greet the patient warmly and invite them to settle in.
- Establishing a Safe Environment: Explain the confidentiality of sessions and that this is a judgment-free zone.
- Session Overview: Briefly outline what the session will involve.
Exploring Feelings (15 Minutes)
- Initial Check-in: Ask the patient how they've been feeling recently and encourage them to express any anxious thoughts or feelings.
- Prompting Questions:
- “Can you describe what anxiety feels like for you?”
- “Are there specific situations or thoughts that trigger your anxiety?”
- Reflective Listening: Paraphrase and validate the patient’s feelings to show understanding.
- Example: “It sounds like you feel overwhelmed by certain thoughts, is that correct?”
Cognitive Behavioral Techniques (15 Minutes)
- Identifying Cognitive Distortions:
- Guide the patient to identify any negative thought patterns contributing to their anxiety.
- Exercise: Use a “Thought Record” to help the patient break down an anxious thought.
- Challenging Negative Thoughts:
- Assist the patient in reframing these thoughts.
- Example: If they think, “I will fail,” help them reframe it as, “I may need to prepare better, but I am capable of succeeding.”
Mindfulness Technique (10 Minutes)
- Guided Mindfulness Exercise:
- Lead the patient through a short mindfulness practice, focusing on breath awareness.
- Instructions:
- “Close your eyes and take a deep breath in through your nose, hold it for a moment, and exhale slowly through your mouth.”
- Encourage them to observe their thoughts without judgment during this exercise.
Closing and Reflection (5 Minutes)
- Summarizing the Session: Recap the key points discussed and the techniques practiced.
- Encouraging Future Engagement: Invite the patient to reflect on how they can bring these techniques into their daily life.
- Assigning Homework (Optional): Suggest that the patient practice the mindfulness exercise at least once before the next session, if comfortable.
Session Notes:
- Remain sensitive to cues of resistance; offer reassurance and encourage the patient to share their thoughts on the therapeutic process.
- Document any significant disclosures or changes in the patient’s mood or engagement during the session for future reference.
Follow-Up:
- Encourage the patient to book their next appointment to continue building on the progress made during this session.
