Psychotherapeutic Session Outline for an Anxious Patient
Session Duration
30 Minutes
Session Objective
To engage the patient in cognitive behavioral therapy (CBT) techniques to address feelings of anxiety and improve overall emotional well-being without focusing on specific issues.
Session Agenda
1. Greeting and Check-in (5 minutes)
- Welcome the patient warmly to create a safe and comfortable environment.
- Briefly inquire about the patient’s week to gauge their emotional state since the last meeting.
- Encourage sharing by asking open-ended questions, e.g., "How have you been feeling lately?"
2. Introduction to CBT Concepts (5 minutes)
- Explain the fundamentals of Cognitive Behavioral Therapy in simple terms:
- Cognition: Thoughts that affect emotions and behavior.
- Behavior: Actions based on those thoughts.
- Emotion: Feelings that can be changed by altering thought patterns.
- Emphasize the goal of identifying anxious thoughts and reframing them.
3. Identifying Anxious Thoughts (10 minutes)
- Ask the patient to describe any recent situations where they felt anxious, despite there being no specific issues.
- Introduce the 'Thought Record' exercise:
- Situation: What was happening at the time?
- Thought: What thoughts were going through your mind?
- Emotion: What feelings did you experience?
- Encourage the patient to identify any cognitive distortions, such as catastrophizing or overgeneralization.
4. Reframing Technique (5 minutes)
- Teach the patient how to challenge anxious thoughts:
- Evidence for the thought: What facts support this thought?
- Evidence against the thought: What facts counter this thought?
- Alternative way to think: How can you view this situation differently?
- Use an example to illustrate this technique and invite the patient to pick one anxious thought to reframe.
5. Closure and Homework Assignment (3 minutes)
- Summarize the key points discussed during the session.
- Assign the patient the 'Thought Record' homework to continue identifying and reframing anxious thoughts throughout the week.
- Encourage the patient to practice self-compassion and note any shifts in feelings as they work on their thoughts.
6. Session Wrap-up (2 minutes)
- Thank the patient for their participation.
- Confirm the next session date and time.
- Encourage the patient to reach out if they have any thoughts or feelings to discuss before the next session.
Session Resources
- Thought Record Worksheet: Provide a template for documenting thoughts, situations, emotions, and reframed thoughts to facilitate the homework assignment.
- Bibliotherapy Recommendation: Suggest a book on CBT techniques for additional reading, such as "The Feeling Good Handbook" by Dr. David D. Burns.
This structured session aims to enhance the patient's engagement in therapy, helping them manage their anxiety through CBT techniques, while remaining open to their experience without focusing on specific issues.
