Psychotherapeutic Session Plan (30 Minutes)
Session Overview
This is a 30-minute Cognitive Behavioral Therapy (CBT) session focused on addressing the patient's feelings of depression and anxiety. The patient currently feels overwhelmed, primarily believing they are responsible for everything that goes wrong in their life. The session will include cognitive restructuring techniques to help challenge this belief and promote a more balanced mindset.
Session Goals
- Help the patient recognize and reframe maladaptive thoughts related to responsibility.
- Increase the patient's sense of self-efficacy and empowerment.
- Provide immediate coping strategies to manage anxiety symptoms.
Session Structure
1. Introduction (5 Minutes)
- Greet the patient warmly and create a comfortable environment.
- Briefly discuss the agenda for the session.
- Assess the patient’s current mood and feelings in the moment.
2. Identifying Negative Thoughts (10 Minutes)
- Ask the patient to articulate any recent situations where they felt responsible for things going wrong.
- Use open-ended questions:
- "Can you think of a recent situation where you felt responsible?"
- "What thoughts went through your mind when that happened?"
- Encourage the patient to express their feelings openly, allowing them to feel heard and validated.
3. Cognitive Restructuring (10 Minutes)
- Introduce the concept of cognitive distortions, particularly focusing on the “personalization” distortion (believing they are responsible for others' outcomes).
- Guide the patient through identifying specific cognitive distortions in their thought patterns.
- Use the following format:
- Situation: Describe a situation where they felt overly responsible.
- Thought: Identify the specific thought they had at that moment.
- Feeling: Discuss what feelings emerged from that thought.
- Reframe: Generate alternative, more balanced thoughts:
- “What would you say to a friend in a similar situation?”
- “Could anything else have contributed to the outcome?”
- Encourage the patient to write down their negative thoughts and the reframed, balanced thoughts.
4. Coping Strategies (5 Minutes)
- Introduce basic coping techniques, such as:
- Breathing Exercises: Teach deep breathing methods to help manage anxiety.
- Mindfulness Practice: A brief guided mindfulness exercise (focus on the breath for 1-2 minutes).
- Suggest that the patient practice these techniques during times of heightened anxiety.
5. Closure and Homework Assignment (5 Minutes)
- Summarize the key points discussed during the session.
- Collaboratively set a simple homework task:
- Encourage the patient to keep a thought diary for the week. This should include:
- Situations that evoke feelings of anxiety or responsibility.
- The negative thoughts associated with those situations.
- Attempts to reframe those thoughts into more balanced perspectives.
- Schedule the next session and express support and encouragement.
6. Concluding Remarks
- Remind the patient that it’s okay to have these feelings but that they don’t have to carry the weight of the world alone.
- Reaffirm the importance of seeking help and sharing concerns in a safe space.
Note
Remember, while this session plan provides a structured approach, it is crucial to remain flexible and responsive to the patient's emotional needs throughout the session. Adjusting the duration or focus on certain areas might be necessary based on patient engagement and comfort.
